Two people have been fired and banned from working in healthcare after faking their medical credentials.
Titilayo Oyedele and Oladele Eluwole, both originally from Nigeria, both worked for NSW Health in pathology roles for a number of years.
An investigation by the Health Care Complaints Commission (HCCC) found they had lied about their experience and pulled the same stunt in the UK years earlier.
Ms Oyedele arrived in Australia in November 2014 and since then has held various positions as a Technical Officer and Hospital Scientist at NSW Health Pathology.
A complaint was made against her, alleging she gave two patients incompatible blood during a transfusion.
NSW Health said the investigation confirmed she was responsible for one of the adverse incidents cited in the original complaint.
"While the error could have been life-threatening, as the patient was in the High Dependency Unit at the time the patient was able to be medically stabilised," a spokesperson said.
Ms Oyedele also lied about employment history when applying to work at NSW Health Pathology, claiming she had held steady employment as a scientist in histology in Nigeria for three years.
She also covered up a similar complaint against her in the UK.
The HCCC determined Ms Oyedele poses a serious risk to the health or safety of members of the public.
She is now permanently prohibited from providing any health services whether on a paid or voluntary basis, and the case has been referred to federal authorities.
Mr Eluwole also arrived in Australia in November 2014, and since then has held various positions in Queensland and New South Wales pathology services.
An investigation was started when HCCC discovered he had been subject to disciplinary action in the UK for providing fake information about his employment history.
The probe also found that Mr Eluwole engaged in similar dishonest and unethical conduct after moving to Australia, claiming to have worked as a scientist in histology in Nigeria for three to four years.
The HCCC determined he poses a serious risk to the health or safety of members of the public.
He is permanently prohibited from providing any health services whether on a paid or voluntary basis, and the case has been referred to federal authorities.
