A grieving mother is searching for answers after her son died following routine appendicitis surgery.

Lucas Peyret went to the Prince Of Wales Hospital in Randwick a year ago, but never came home.

Instead, the 21-year-old suffered brain damage during the procedure and died 10 days later.

"He was a beautiful soul, still is," his mother Katya Denomme told 9News.

When Mr Peyret went to the Sydney hospital, his abdominal symptoms were monitored.

"The doctor told me it was indeed appendicitis and he would have surgery maybe the following morning because it wasn't urgent," Ms Denomme said.

"He didn't have any fever or very strong pain."

Instead, an emergency laparoscopic procedure was performed that Sunday night, which was supposed to be uncomplicated.

Mr Peyret was given sugammadex to speed up his recovery from muscle relaxant drugs, but within minutes he deteriorated and went into cardiac arrest.

Ms Denomme was told her son suffered a rare reaction to the drug, which is widely used in Australia.

Ten days later he died, and his heartbroken mother told 9News she never received an apology from the hospital, and his cause of death is still unclear.

"Once something like this happens it's shocking," she said.

"I hired a lawyer to find the truth and to prove medical negligence."

A hospital report revealed four things:

In a statement the hospital said its analysis "identified some opportunities for system improvement ... all of which have been implemented" and "the report did not conclude there was any delay in providing resuscitation".

The hospital also said it extends its sincerest sympathies to Mr Peyret's family, but his mother said she is yet to receive a formal apology.

"I just don't want this to happen again," Ms Denomme said.
